We use tailor-fit assessment tools to evaluate each resident’s capabilities and strengths so that the program can best meet their needs.

The effectiveness and success of our Spark® program for Assisted Living and Independent Living residents is based on 4 foundational pillars.

• Social: We ensure that each resident has a role in the community that allows them to interact with others.
• Skills Focused: Our Montessori Assessment Tool uncovers remaining skills often missed by traditional evaluations.
• Procedural Memory: Skills can be learned and even improved through repetitive “procedural learning,” which allows residents to perform routine activities with less concerted effort.

Health & Wellness Services

Whether you or your loved one requires some help with the activities of daily living (ADLs), our dedicated staff works hard to deliver high-quality health and wellness services that are compassionate, safe, and resident-centered. Our services are designed to meet our residents’ physical, emotional, and spiritual needs 24 hours a day, seven days a week.

Before residents move to our Lebanon community, customized assessments are completed to create individual care plans. Our community of highly trained medical professionals, including EDs and RNs, develop plans and are hired for their professional skills and their compassionate and warm personalities.

Accommodations & Amenities

Our community features spacious living quarters and private studio, one and two-bedroom rooms. Choose from kitchenettes or full kitchens. Wi-Fi and cable are available. Take advantage of the meeting room for special presentations, music, movies, and other events. Residents also enjoy our library, game room, large library, pretty garden area with water features, and small bridge.

At The Oaks at Lebanon, we love food and create many new, exciting recipes for our residents. We’re famous for our homestyle recipes. We also host a fish fry and serve delicious dinners all year, especially during the holidays when prime rib and salmon are the main entrees. Friends and family are always welcome.
